The Athletic Training staff welcomes you to the opportunity to try out for one of our athletic teams. Before your tryout, you must complete the following paperwork. Failure to complete the paperwork prior to your tryout date will result in no participation.
- Physical (A physical must be completed within the last 6 MONTHS of your try out)
- Sickle Cell Waiver (Print this form and sign it)
- Copy of front and back of insurance card
- Assumption of Risk (This is located on the bottom of On-Campus Tryout Form. The coach will provide this form)
As a reminder, the physical must be within
6 MONTHS of your tryout date. This is a strict policy that is enforced.
